Travel Advice & Questionnaire

Warning
As of March 20th 2025 we are currently not able to provide the following travel vaccinations. Please note due to capacity we need at least 8 - 10 weeks notice. If your request is less than the minimum 8 week period we may have to refer you to a local travel clinic.
* Rabies
* Hepatitis B
* Japanese encephalitis
* Tick-borne encephalitis
* Yellow Fever
These need to be sourced at a local travel clinic such as Boots in Truro/Fraddon etc.
We offer routine vaccinations and general advice to our patients travelling abroad.
It may take up to eight weeks for a full course of vaccinations, so please contact us in advance to give you plenty of time if you’re planning to go abroad where you need to be vaccinated. We need at least 10 weeks notice.
Please visit - www.travelhealthpro.org.uk
Enter your destination details and this will give you up to date travel health advice for that location including recommended vaccinations and if malaria protection is advised.
Please ensure you complete this step before the online questionnaire below.
We need you to research the vaccinations you might require and list them on the form, even if you are unsure if you have had that vaccination in the past. The website also includes really useful information about your travel and the destination. Without completing this step and researching what vaccines you may need your form will be returned to you and we will be unable to book an appointment with our nurses.
