You can register online to become a new patient at Carnon Downs Surgery. Please note you should complete both the Registration Form and the Health Questionnaire.

If you are a new patient and would like a copy of our practice leaflet please ask at reception.

How to register an Adult as a new patient

Complete the BOTH the online form to register and the health questionnaire below.

You will need to fill in all of the details to register at our surgery on both forms.

If you complete the forms at the desk you will also need to provide a copy of your ID and proof of address. For example, valid ID could include a photograph of your passport or driving licence. This is to check that you live in the local catchment area.

We may also ask you for details of your previous GP surgery and basic health and medical information.

How to register a Child as a new patient

Children under 16 need to be registered by their parent or guardian.

You can do this by completing the online GP surgery registration form and the child health questionnaire form below, on their behalf.

You will need to provide a form of ID to prove that you’re their parent or guardian.

Our catchment area

You can use your postcode or location to check that you’re within the surgery’s catchment area. 

Check your postcode on the Find a GP page on the NHS website 

If you live outside of our boundary but wish to register, as of 1st January 2022 we will no longer be accepting registrations for patients who live outside the marked area. This also includes patients who currently are on the 'Out of Area' scheme and move to another address which is still outside our boundary. 

This also includes patients returning home after studying and living away at university, they will now have to be seen at a practice nearer their location along with new babies that are born to parents who reside outside the practice area.

Registering as a temporary patient

If you are away from home, you can register as a temporary patient with Carnon Downs Surgery for up to three months.

This will allow you to use our surgery and still remain a patient at your permanent GP surgery. After three months, you will have to re-register as a temporary patient or permanently register with us.

What to do if you’re registering on behalf of someone else

If you are a carer of young children, elderly adults or adults who aren’t able to register themselves, you can set up an account on their behalf. This will allow you to book an appointment and access parts of their GP online services account.

Change of Patient Details

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you will be asked to register with a surgery that’s closer to your new home address.

You can contact us online to let us know about a change in your address or contact details.

Please also let us know if your name,  telephone number or email address changes.

Registering online as a non-English speaker

You can find guidance and advice on how to become a new patient and the NHS healthcare services you can access, if your first language is not English, or if you’re an asylum seeker on the Government website.

Named Allocated GP

As part of our serivce to you, we would like to inform you that you have a named allocated GP which is recorded on your medical record. If you would like to know who your named allocated GP is, please ask Reception. This does not mean that you cannot see a GP of your choice as we do not hold individual GP lists and you are registered with the Practice in general.
